Access Request Details

If you only wish to manage your appointments or order repeat medications/appliances, you will be emailed a password for you to register to start using the online service. If you aren’t registered with us yet, your code will be emailed to you within three weeks, prior to the form being filled in and photo identification being provided.

Patient Declaration

If you also wish to view detailed coded information in your medical record online, your record needs to be checked by your doctor first and your password will be emailed within 3 weeks. Please first read and consider the below. Forgotten history - There may be something you have forgotten about in your record that you might find upsetting. Abnormal results or bad news - If your GP has given you access to test results or letters, you may see something that you find upsetting to you. This may occur before you have spoken to your doctor or while the surgery is closed and you cannot contact them. Choosing to share your information with someone - It’s up to you whether or not you share your information with others – perhaps family members or carers. It’s your choice, but also your responsibility to keep the information safe and secure. Coercion - If you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time. Misunderstood information - Your medical record is designed to be used by clinical professionals to ensure that you receive the best possible care. Some of the information within your medical record may be highly technical, written by specialists and not easily understood. Detailed coded information is Immunisations (all), Laboratory test results (last 10 years onwards), Documents (last 10 years onwards), Problems (last 10 years onwards), Consultations (from now onwards) Please first read and consider the below.


Patient Identification Verification

We need to check your identification before you can use Patient Access. Please send a copy of your official photo ID (Passport/ Driving Lisense/ National ID Card) to NCLICB.TMHP.registrations@nhs.net 

Please note your application will NOT be processed if you do not provide photo ID, and the form will be discarded in 1 week.
